Some children and young people (up to age 18), may have very complex health needs.
These may be the result of:
- Congenital condition
- Long-term or life-limiting condition
- Disability
- Serious illness or injury
Children and Young People’s Continuing Care is a service to support parents or carers to meet their child or young person’s health needs because they cannot be met by existing universal and specialist services.

Meet The Children and Young People Continuing Care Team
Who can refer into our service?
Children and Young People’s Continuing Care team can only receive referrals from health professionals or social care colleagues. Any referral received must be accompanied by a signed consent form. Without consent, the referral will be rejected.
What happens next?
Once both the referral form and consent form have been received the information contained within these forms will be triaged by the children’s continuing care nurse assessor who will make a clinical decision as to whether we proceed to a full assessment. The person who submitted the referral will be informed of this outcome.
Who is eligible?
Any child or young person up to their 18th birthday who has complex health needs that are not met by universal services may be eligible for Children and Young People’s Continuing Care funding. When a young person reaches 18, they will require an Adult NHS Continuing Care referral as the criteria for adults is different to children and young people’s continuing care.
What happens if my child does not meet the criteria?
We understand it can be difficult for families to hear that their child does not meet the criteria. We will fully explain the reasons why your child does not meet the criteria and is therefore not eligible for children and young people’s continuing care funding.

Reviews
The first review will take place three months after the package of care has started. In most cases, reviews are then undertaken annually. However, if there are changes to your child or young person’s clinical health needs, we can review the package of care on a more frequent basis.
Transitioning to Adult’s Continuing Healthcare
In preparation for adulthood, if a child or young person is in receipt of a package of care children and young people’s continuing care, a referral needs to be made to the Adult Continuing Healthcare Team, this is usually made by Social Care as part of the transition process.

Complaints
If following a discussion with the team, you remain dissatisfied, further support and information can be obtained on how to raise a complaint via NHS Gloucestershire’s ICB Patient Advice and Liaison Service (PALS). Further information can be obtained on the complaints page.
Appeals
You have the right to appeal if:
- You believe that your child’s continuing care assessment was not completed accurately and/or it has not been explained to you properly.
- You believe that the Children and Young People’s Continuing Care panel have not followed the correct process.
