DSR-Children, Young People and Transitions

The Dynamic Support Register for Children and Young People in Gloucestershire is managed on behalf of the NHS Gloucestershire ICB by Gloucestershire Dynamic Keyworker Service led by Service Lead for the Keyworker Function & Care & Education Treatment Review. Once your child has been identified and added to the DSR, their care will be routinely reviewed in combination with their support network to make sure that all of the best practice for young people with a Learning Disability and/or Autism is in place. We know that the needs of young people can change quickly, so this register is intended to aid early identification of increased risk and help us to proactively support them in the community.

Families or Carers

Please complete a Consent Form as well as the DSR Register Form (DSR1)


Professionals

Please complete a signed Consent Form with the family (and indicate if signing on their behalf). Complete the DSR Register Form (DSR1).

We also require a Dynamic Assessment Tool (DAT) from a professional. Please refer to the guidance on completing this.
