DSR for Adults

The lead person with responsibility for the maintenance and oversight of the DSR for people over 18 years of age will be the Senior Commissioning Manager (LD/ASC) & Transforming Care Programme Lead.

Notification to the DSR could be from Professionals. People with LD/ASC/families/unpaid carers/others will be able to self-refer for inclusion on DSR.

Once the notification or form for self-referral is received, the person will be assessed against set criteria. If eligible to add to the DSR, the notifier or the referrer will be required to seek consent from the individual.
